US helicopters in action

Has there ever been a case when the US admitted that one of its helicopters was actually downed in Iraq or Afghanistan (or Pakistan) by enemy gunfire?  It is always mechanical error.  They always forget to check the oil in the helicopter before they embark on a mission.
